Registration Forms

For membership, all competitors should fill in the following form on a yearly basis:

Swimming Test Form

All competitors registering for the first time to CREBA, should fill in the following form as a proof for swimming profiency:

Escort Boat request form

All teams that need an escort boat from the Club to practice the Gatun Lake Stretch, are required to fill in the following form and bring it to the Club Office. This document should be brought to the club office, the latest, the Friday before the Lake practice.
