How to enroll to CREBA

To be eligible into CREBA, you must complete the following steps:

• Must complete and submit the registration form available at the CREBAs Office or from Forms Download Section here).

• You must to complete and submit the waiver form and liability waiver Download here.

• You must deposit at Banco General the amount of $50.00 annual membership fee and deliver the slip to the CREBAs Office in order to proof the deposit. (Banco General, Current Account Number: 03-38-01-037626-9)

• You must present a birth certificate (proof of age) and health certificate at the CREBAs Office.
